Expanded In-Home Services for the Elderly

Program Details

This program serves Schuyler County residents age 60+ who live independently but need help with personal care or household tasks. It is for those not eligible for Medicaid and unable to afford private care. Services are cost-shared based on income, with some clients paying nothing, and may include a PERS for in-home emergencies.

Services

EISEP in-home services are delivered by contracted home care agencies and include help with tasks such as housekeeping, bathing, grooming, and changing linens. Service hours are determined by an OFA case manager based on an assessment of needs.

Ancillary Services

Individuals may have unique needs to help them remain safely at home, such as assistive technology, adaptive equipment, personal supplies, or a PERS for one-touch access to emergency help. EISEP can assist with meeting those needs.

Case Management

EISEP provides case management to assess needs, develop and coordinate a service plan, and monitor progress to ensure individuals meet their goals and safely remain in the community.

All services are subject to budget limitations and availability.
